Below is the link to our New Client Forms containing pertinent client information and financial policies. Please take time to read this information so you will be aware of standard office procedures and charges. If you have any further questions, our front office will be glad to assist you.

If you have not already done so, please verify the following:

• Insurance coverage (deductible, co-pay - the % client is to pay)
• Number of allowed visits for mental health
• Initial authorization/pre-certification (If you fail to do so, it may result in denied coverage by the insurance. In this case, you would be responsible to pay charges not covered by your insurance company.
• Obtain the mental health claims address for your insurance
